APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Nuclear Generating Station Docket Nos.
50-272 50-311 License Nos. DPR-70 DPR-75 As a result of the inspection c6nducted on April 7-10, 1987, and in accordance with the NRC Enforcement Policy, (10 CFR Part 2, Appendix C). the following violations were identified:

10 CFR 50 Appendix B, Criterion V requires that *activities affecting quality shall be prescribed by documented.instructions, procedures, or drawings and that they shall include appropriate quantitative or qualitative acceptance criteria for determining tha~ important activities have been satisfactorily accomplished.
The licerisee 1 s Quality Assurance program incorporated 10 CFR 50, Appendix B prior to issuance of Bulletin 80-11, Masonry Wall Design.
Contrary to the above, as of April 10, 1987, a written procedure providing the scope and the quantitative and qualitative acceptance criteria was not documented for the 1980 walkdown survey of block walls performed in re-
. sponse to NRC Bulletin 80-11 to determine their location in relation to safety-related equipme~t.
This is a Severity Level V violation. (Supplement 1) 2.. Criterion XVII of Appendix B, 10 CFR 50 requires that sufficient records be maintained to furnish evidence of activities affecting quality, and the licensee's Engineering Directive EDD #4, Revision 1, requires that calculations shall record the structural adequacy of design and be maintained for Design History.
Contrary to the above the licensee did not have recorded, controlled ca lcul ati ons demonstrating the structural adequacy of.a 11 masonry walls and of the modifications which had been made to them in response to NRC Bulletin 80-11.
This is Severity Level V violation.

Pursuant to the provisions of 10 CFR 2.201, Public Service is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid. further violations; and (3) the date when full compliance will be achieved..
Where good cause is shown, consideration will be given to extending this response time.
