APPENDIX A NOTICE OF DEVIATION Public Service Electric & Gas Co.
Salem Nuclear Generating Station Docket Nos. 50-272 50-311 License Nos. DPR-70 DPR-75 As a result of the inspection conducted on dance with the General Statement of Policy Actions, 10 CFR 2, Appendix C (Enforcement were identified:
September 8-12, 1986, and in accor-and Procedure for NRC Enforcement Policy 1986) the following deviations

Action Item No. 1 of the IE Bulletin No. 82-02 specifies that Quality Assurance measures should be established for proper selection, procurement and application of fastener lubricants to Reactor Coolant Pressure Boundary (RCPB) threaded fasteners to minimize fastener susceptibility to stress corrosion cracking (SCC) environments.
The July 29, 1982 reply by the licensee to Action Item 1 of IEB 82-02 provides that all necessary bolting practices are accomplished in accordance with written procedures, and QA measures are established within the procedure along with appropriate QA signoffs.
Contrary to the above, the quality controls committed to by the licensee for the designation, purchase, receipt inspection, verification and documentation of two lubricants, Neolube and FELPRO N5000, used on RCPB threaded fasteners were inadequate in that:
For Neolube:
The associated purchase orders did not clearly designate the specific product to be purchased.
The maintenance procedure under which Neolube is used, M8C-3, provided an identifying number that when traced through the purchase order and receiving process to the point of issue resulted in either Neolube 1 or 2 being available for issue from the maintenance storeroom.
Neolube 2, the material designated by the NSSS supplier for use on the reactor pressure vessel stud holes, is not certifiable by the manufacturer with respect to halogens.
The purchase order designates this lubricant as 11 not safety related 11 and does not require a certificate of chemical analysis for elements that may contribute to SCC of RCPB fasteners.
Chemical analysis results were not provided with either Neolube 1 or 2 and no samples received at the site were analyzed to verify acceptability.

Receiving inspection documentation indicated only a verification that the number of containers ordered were received.
For Felpro N5000:
This lubricant was designated as 11 not safety related 11 on its purchase order, with the result that receiving inspection did not verify the product quality but only verified the number of containers received.
The purchase order did not require a chemical analysis certification and, although a chemical certification was received with two batches of Felpro N5000, no provision was made for its review and acceptance.
Further, no documentation indicating that a review of the chemistry analysis was performed could be found.
Maintenance procedure and contractor work package documentation reviewed did not demonstrate that an acceptable product was verified to have been actually used.

Action Item 4 of the IE Bulletin No. 82-02 specifies that a written report signed under oath be submitted to the Regional Administrator within 60 days following completion of the outage during which action item 2 of the Bulletin was performed.
Contrary to the above no written report as specified by Action Item 4 was issued by the licensee.
The Public Service Electric and Ga:s Company is hereby requested to submit to this office within twenty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further deviations; and (3) the date when full conformance will be achieved.
Where good cause is shown, consideration will be given to extending this response time.
