UNIT NOS. 1 AND 2 DOCKET NOS. 50-272 AND 50-311 Public Service Electric and Gas Company (PSE&G) is in receipt of your letter dated October 17, 1985 which transmitted a Notice of Violation regarding a failure to establish procedures in accordance with Technical Specification 6.8.
Pursuant to the provisions of 10 CFR 2.201, our response to the Notice of Violation is provided in Attachment 1.

10 CFR 2.201 INFORMATION PUBLIC SERVICE ELECTRIC AND GAS SALEM GENERATING STATION RESPONSE TO NOTICE OF VIOLATION Your letter dated October 17, 1985 identified two Violations of the Technical Specifications.
As stated therein, the Violation was the result of a failure to maintain procedures in accordance with Technical Specifications in that Surveillance procedure SP (0) 4.4.6.2.d (Reactor Coolant System (RCS) Water Inventory Balance) was inadequately established.
This resulted in leakage occurring inside the containment to be classified improperly, and hindered entry into the Limiting Condition for Operation for Technical Specification 3.4.6.2 for events where the "unclassified" leakage would have increased the unidentified leakage above 1.0 gallons per minute
( GPM).

PUBLIC SERVICE ELECTRIC AND GAS DOES NOT DISPUTE THE VIOLATION

THIS EVENT WAS ATTRIBUTED, IN PART, TO PERSONNEL ERROR BECAUSE OF THE FAILURE TO FOLLOW PROCEDURES AND TO ENTER THE ACTION STATEMENT WHEN UNIDENTIFIED CONTAINMENT SUMP IN-LEAKAGE EXCEEDED 1.0 GPM.
THE ROOT CAUSE OF THE VIOLATION WAS THE FAILURE TO PROVIDE ADEQUATE PROCEDURAL GUIDANCE CONCERNING THE QUANTIFICATION OF PRIMARY TO CONTAINMENT ATMOSPHERE LEAKS.

IMMEDIATE CORRECTIVE ACTIONS:
a)
The supervisors involved with this occurrenqe were counselled concerning their actions during the event.
b)
The policy concerning the handling of operational leakage events was re-emphasized to other Operations Department personnel in the daily newsletter.
c)
Operating Instruction OI II-1.4.5 (Reactor Coolant Leak Detection) and Surveillance Procedures SP(O) 4.4.6.2 (Unidentified/Now Identified Leak Rate Determination) and SP(O) 4.4 ** 6.2.d (RCS Water Inventory Balance) were revised to provide sufficient operator Guidance for identifying and quantifying steam leaks.

LONG TERM CORRECTIVE ACTIONS:

a)
Consistent with PSE&G's policy concerning personnel error related events, a discussion of this event is being included in the appropriate training programs; specifically, the "Plant and Industry Events" section of the Licensed Operator Requalification Program (Segment 2).
This training commenced on November 12, 1985, and will be completed by January 31, 1986
- FULL COMPLIANCE WILL BE ACHIEVED BY JANUARY 31, 1986.
