Combined Inspection 50-27~50-311/85-24

This refers to the routine resident safety inspection by Mr. T. Kenny and Mr.

R. Borchardt, of this office, on October 1, 1985 - October 31, 1985, at the

Salem Nuclear Generating Station, Hancocks Bridge, New Jersey.

The inspection

consisted of document reviews, interviews, and observation of activities.

The

findings were discussed with Mr. J. M. Zupko, Jr. of your staff.

Within the scope of this inspection no violations were observed; however, we

believe that increased management attention is warranted in the operation of

the Waste Gas Analyzer System.

The most recent event involving this system was

a failure to sample the inservice waste gas decay tank continuously as required

by Technical Specifications. This event is discussed in Licensee Event Report

272/85-10 and paragraph 10 of the enclosed report.

It should also be noted

that an improper on-the-spot change to a Waste Gas Analyzer System procedure

was recently cited as a violation.

The root cause of these Technical Specifica-

tion violations appears to be inadequate training and a general lack of under-

standing of how the new Waste Gas Analyzer System works.

This system was

recently_ installed in order to satisfy Technical Specification sampling require-

ments, but it became apparent that the personnel responsible for its operation

did not fully understand the design and operating principles.

Your proposed

corrective actions discussed in Unit 1 LER 85-10 appear adequate for this

specific case; however, your actions should be broadened to insure that all

newly installed systems are understood thoroughly by the operators and that

training is conducted to insure successful operation of the system.
