APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Hancocks Bridge, New Jersey Docket No.
50-311 License No. DPR-75 As a result of the inspection conducted on March 21-22 and March 25-29, 1985, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), pub-lished in the Federal Register on March 8, 1984 (49 FR 8583), the following violation was identified:
10 CFR 50, Appendix B, Criterion II Qua 1 ity Assurance Program states in
- part, 11The quality assurance program shall provide control over activities affecting the quality of the identified structures, systems, and compon-ents, to an extent consistent with their importance to safety. 11 PSE&G Nuclear Operation Quality Assurance Manual Procedure QAP 5-3, 11 In-spection Program 11 states in part, 11 Inspection Notification Points shall be used as a minimum for work delineated in Attachment 1 of that document unless waived by the Station Q.A.
11 QAP 5-3 Inspection Program, Attachment I states in part, "The following Inspection Hold Points may not be waived except by Site Quality Assurance Engineer.
11 One of these hold points is verification of bolt torque on foundations and seismic restraints.
Contrary to the above, on March 28, 1985, it was identified that the 2C battery support rack, a structure important to safety, was installed with-out quality assurance control.
Specifically, the 2C battery support rack
- (which was installed to seismic qualifications described in Design Change Request 2EC-2003) did not conform with the provisions delineated in QAP 5-3 in that there were no inspection hold points in the installation procedure and, no waivers were documented to a 11 ow for the 1 ack of inspection hold points.
Collectively the above constitute a Severity Level IV Violation (Supple-ment I).
Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) the corrective steps that will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
