COMBINED INSPECTION 50-272/82-06 and 50-311/82-05 SALEM GENERATING STATION UNITS NO. 1 AND 2 FEBRUARY 9 THRU MARCH 8, 1982 We have reviewed the items of violation which were identified in the subject inspection and described in Appendix A of your letter dated March 18, 1982.

The following are our responses to those identified items:

ITEMS OF VIOLATION Item A 10 CFR 55.33(b) extends the term of operator licenses based on submittal of renewal applications at least 30 days in advance of expiration.

The operator at the controls of the facility must be licensed pursuant to Part 55 as re~uired by 10 CFR 50.54(k).

Contrary to the above:

During the ~eriod May 17, 1981 through February 1, 1982, the operator licensed under Operator License OP-4433, was frequently the operator at the controls.

The license ex-pired on May 17, 1981, and application for renewal was not made until September 25, 1981.

Mr. Ronald c. Haynes, Director U.S. Nuclear Regulatory Commission Item B 4/16/82 Technical Specification 3.7.11 requires that, with fire barrier penetrations inoperable, either a fire watch be stationed at one side of the penetration or a fire patrol be established once operability of the fire detectors in the areas has been confirmed.

Contrary to the above:

1)

On February 15, 1982, two unsealed 4" core bores were found between the electrical and mechanical penetration areas on elevation 78'.

No fire watch was present on either side of the open fire zone boundary penetration and no fire patrol had been established.

2)

On February 23, 1982, the door in the fire boundary separating the Unit 1 and 2 4KV switchgear rooms on elevation 64' was propped open.

No fire watch or fire patrol had been established on either side of this open penetration.

Reply to Item A During the period of May 17, 1981 through February 1, 1982, the operator licensed under Operator License OP-4433-2 was frequently the operator at the Control Board because the Station was under the impression that his license had been renewed.

Upon notifi:....

cation that the renewal had been refused by the NRC, he was immediately removed from licensed duties.

The error occurred during the license renewal process.

In the computer scheduling system, six months prior to license expiration a card is issued from the computer to insure that the individual is scheduled for a physical examination.

Three months prior to license expiration another card is issued which directs that the license renewal application be prepared.

Normally, the application for renewal is submitted six weeks prior to license expiration so that the existing license remains in effect until the NRC acts on the renewal application.

In January, 1981, the operator was examined by the NRC-OLB for Salem Unit 2 RO License.

He failed that examination, and sub-sequently, in order to obtain additional training, was removed from licensed duties on Unit 1.

During that time frame the Licensed Operator Roster was revised, and because he was off licensed duties, his name was inadvertently omitted from it.

The revised roster was utilized to input data to the computer.

Therefore, when the cards were issued for the May 17, 1981 group of license renewals, his license was not triggered for renewal.

When Training cross-checked the renewal applications against the roster, there were no discrepancies.

Meanwhile, he had been restored to licensed duties on Unit 1.

It was then, on September 18, 1981, that the discrepancy was discovered.

A lice.nse renewal application was submitted on September 25, 1981.

On February 1, 1982, the application was rejected by the NRC, and the operator was removed from licensed duties.

The license renewal process has been modified to the extent that the Licensed Operator Roster is now generated directly from the licenses, and grouped according to renewal date.

This action will preclude any similar recurrence.

Reply to Item B The failure to establish a fire watch or patrol on February 15 and 23, 1982, was due to oversight.

Prior to these two occurrences, the areas which posed problems in controlling, tracking and posting of fire watches had been identified.

As a result of the investigations that were con-ducted in these areas, recommendations were made to correct the problems.

The recommendations were being adopted at the time of the occurrences, and a new roving fire watch program was developed and implemented on a trial basis. --------

The new program provides four roving fire watches to patrol the safety related areas of the station.

These watches patrol at least one side of all inoperable fire barriers once per hour.

The roving fire watches work under the supervision of the Shift Support Coordinator (SSC).

The SSC also has the responsibility for controlling and tracking all fire door inspedtions and th~

openings and closures of fire barrier penetrations.

Provisions for the posting 0£ continuous fire watches on inoperable fire barriers has also been -provided for at the direction of the SSC, when needed.

Full implementation of this program is expected by May 1, 1982.

At that time we will be in full compliance.
