During inspection on March 9 - April 59 1982, the fol lowing violation was identified and categorized under the NRG Enforcement Policy, 10 CFR 2
- Appendix C (47FR9987, March 9, 1982).
A.1 Technical Specification 6.8.1 and Regulatory Guide 1.33 require implementation of procedures for safety related tagging. Licensee procedure AP-15, Safety Tagging Program requires the work supervisor to ensure the equipment is isolated completely and that all tags are properly placed before starting work.
Contrary to the above; On March 15, 1982, a tag issued to isolate valve 1 RH 26 in Unit 1 was found on the breaker for valve 2 RH 26 in Unit 2, with the valve motor de-energized.
On March 26~ 1982, Safety Injection Pump 12 was isolated by tagging the motor breakers for valves 12SJ33 and 11SJ45.
No tags were placed on the Limitorque manual operators to preclude local opening of the valves.
This is a Severity Level IV Violation {Supplement I) applicable to DPR-70 and DPR-7q Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
{l) the corrective steps which-have been taken and the res~lts achieved; (2) corrective steps which will be taken to avoid further viola-tions; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown9 consideration will be given to extending your response tim~.
