Public Service Electric and Gas Company APPENDIX A NOTICE OF DEVIATION Docket No. 50-311 License No. DPR-75 Based on the results of an NRC inspection conducted during the period July 29-31, 1981, it appears that one of your activities was ~ot conducted in conformance with your commitment to the Commission as indicated below:

Section 13.6.2 of the Final Safety Analysis Report (FSAR) states, 11All tests are performed in strict accordance with approved written test procedures... A detailed step-by-step procedure is provided for each
.test.
11 Section 10.1.5 of Startup Procedure No. 81.13, Radiation Monitoring and Shielding Evaluation, developed pursuant to the above, re-quires 11 QA notification signoff 11 after the radiation survey is completed.
Contrary to the above there was no QA notification signoff after the radiation survey was completed at zero power on August 2, 1980, neither was there a QA notification signoff after the radiation survey was completed at 30 percent power on July 1, 1981.
Public Service Electric and Gas Company is hereby requested to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply including the corrective steps which have been taken and results achieved (or corrective steps that are planned), and the date when corrective action will be completed.
Where good cause is shown, consideration will be given to extending your response time.
The response directed by this Notice is not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
