This refers to the inspection conducted by Messrs. K. Plumlee and R. Nimitz of

this office during the period October 7-9 and 14-17, 1980 of activities authorize9

by NRC License No. DPR-70 and to the discussions of our findings held by Mr .

. Nimitz with Mr. H. Midura, Manager - Salem Generating Station of your staff at

the conclusion of this inspection and to a letter dated October 24, 1980, from

this office to you following this inspection.

The enforcement correspondence

pertaining to this inspection has been provided separately to Mr. R. M. Eckert,

Senior Vice President - Energy Supply and Engineering, from NRC Headquarters, by

the Director, Office of Inspection and Enforcement.

In accordance with Section 2.790 of the NRC 1 s

11 Rules of Practice,

11 Part 2, Title

10, Code of Federal Regulations, a copy of this letter, the enclosed inspection

report and enclosed inspection report supplement will be placed in the NRC 1 s

Public Document Room.

If this report contains any information that you (or your

contractor) believe to be proprietary, it is necessary that you make a written

application within 20 days to this office to withhold such information from

public disclosure.

Any such application must be accompanied by an affidavit

executed by the owner of the information, which identifies the document or part

sought to be withheld, and which contains a statement of reasons which addresses

with specificity the items which will be considered by the Commission as listed

in subparagraph (b) (4) of Section 2.790.

The information sought to be withheld

shall be incorporated as far as possible into a separate part of the affidavit.

If we do not hear from you in this regard within the specified period, the

report will be placed in the Public Document Room.

No reply to this letter is required; however, should you have any questions

concerning this inspection, we will be pleased to discuss them with you.

  • These personnel attended the exit interview, conducted at 1 PM on October

17, 1980.

Inspection Purpose and Scope

The purpose of this special inspection effort was to review the circum-

stances and licensee followup and corrective actions following personnel

entry into unknown high radiation and/or contamination areas. The first

event reviewed was entry of two individuals into the Unit 1 Fuel Transfer

Tube on October 4, 1980.

The two individuals sustained extensive personal

contamination and limited intakes of radioactive material.

The second

event reviewed was the entry of three individuals into the reactor flux

thimble area under the Unit 1 reactor vessel on October 10, 1980.

The

individuals unknowingly entered a very high radiation field emanating from

the retracted flux thimbles and received higher than anticipated radiation

exposure.
