. "APPENDIX A
.. NOTICE. OF "VIOLATION Public Service Electric and Gas Canpany Docket No. 50-272 Based on the results of an NRC inspection conducted on October 12 through November 15, 1980, it appears that one of your activities was not conducted in full compliance with the condition of your NRC Facility License No. DPR-70, as indicated below. This item is categorized as an Infraction.
Technical Specificati.on 6.12.1 (b) states in part:
"A high radiation area in which the intensity of radiation is greater than 1000 mrem/hr shall be subject to the provisions of 6.12.1.a, above, and in addition locked doors shall be provided to prevent unauthorized entry into such areas *** "
Contrary to the above,. the following instances of unlocked access doors into.
high radiation areas were identified.
- a. At 12:45 p.m. on November 3, an access door located on elevation 55' in the Auxiliary Building was found open and unattended. The door provides access to elevations 55' arid 45' where the Auxiliary Building Drain Tank and the Residual Heat Removal components are located, Radiation levels in the area around the drain tank exceeded 1000 mrem/hr as confirmed by the most recent survey.
- b. At 7:.00 a.m. on November 7, an access door located on elevation 100' in the Auxiliary Building was found open and unattended. 1he door provides access to the radioactive 1 iquid waste processi_ng area.
