Dear Sir:

COMBINED INSPECTION 50-272/a0-23 and 50-311/80-18 We have reviewed the report of the subject inspection conducted by Mr. L. Norrholm of your office on October 12 through November 15, 1980.

Our response to the infraction identified. in Appendix A is as follows:

Item of In*fraction Based on the results of an NRC ins;pedtion conducted on October 12 through November 15,.1980,.it appears that one of your activities was not conducted in full compliance with the condition of your NRC Facility License No. DPR-70, as indicated below.

This item is categorized as an Infraction.

Technical Specification 6.12.l(b} states in part:

"A high radiation area in which the intensity of radiation is

  • greater than 1000 mrerri/hr shall be subject to the wrovisions.

of 6.12.1.a, above, and in addition locked doors shall be provided to prevent unauthorized entry into such *areas *** "

Contrary to the above, the following instances of unlocked access doors into h~gh radiation areas were identified.

At 12:45 p.m. on Noverriber 3, an access door located on

elevation 55' in the Auxiliary Building was found open arid unattended.

The door provides access to elevations 55' and 45' where the Auxiliary Building Drain.Tank and the Residual Heat Removal components are located.

Radiation levels in

  • the area around the drain tank exceeded 1000 mrem/hr as confirmed by.the most recent.survey *

At 7:00 a.tn. on November 7, an access door located on* ele-vation 100' in the Auxiliary Building was found OJ?en and unattended.

The door provides access to the radioactive liquid waste processing area.

Radiation levels in the processing area exceeded 1000 mrem/hr as confirmed.by the.

most recent survey.

The Auxiliary Building elevation 55' access door was ap-parently left open due to a positive ventilation flow and a weak door.closure device which the last person to.exit.the area was not aware of.

2.

The door was immediately closed.

3.

A new door closing device is being ordered which should prevent the recurrence.

Until the new device is installed, a sign will be posted to notify those using the door that they must ensure it is latched closed after use.

4.

Door closure device will be installed by April 1, 1981.

The Auxiliary Building elevation 100' radioactive *liquid waste processing area access door was left open by a con;..

tractor operator while taking a radioactive sample to the Chemistry Laboratory.

2.

The door was immediately closed.

3.

The individual was reminded of the importance *of locking high radiation area doors and disciplinary action was

  • administered.
4.

We are in compliance now.
