Auxiliary Feedwater Storage Tank (AFWST)

< 200,000 Gallons CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 Low Power Physics Testing DESCRIPTION OF OCCURRENCE:

During routine surveillance of the control console, it was determined the level in the AFWST was below the minimum level (200,000 gallons).

Action Statement 3.7.1.3 was entered at 1349 hour0.0156 days <br />0.375 hours <br />0.00223 weeks <br />5.132945e-4 months <br />s-which requires the level to be restored within four hours or be in Hot Shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel Error ANALYSIS OF OCCURRENCE:

In the process of using the AFWST as a makeup supply of water to the steam generators, the supply valve for the AFWST was throttled too far to compensate for the water removed.

CORRECTIVE ACTION

The tank was refilled to> 200,000,gallons of stored water and the Action Statement was terminated at 1421 hours0.0164 days <br />0.395 hours <br />0.00235 weeks <br />5.406905e-4 months <br />.

A memo will be sent to all licensed operators to heighten their awareness of the narrow band of operation permitted by Technical Specifications.
