08038 IDENTIFICATION OF OCCURRENCE:

Rod Position Indication Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 DESCRIPTION OF OCCURRENCE:

While withdrawing rods -in preparation for entering Mode 2, 1SC2 individual rod position indicator read 20 steps lower than the bank demand position.

Action Statement 3.1.3.2.2 was entered at 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> which requires that with a rod position indicator inoperable, immediately open the reactor trip breakers.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure ANALYSIS OF OCCURRENCE:

When it was determined that the individual rod position indicator was reading inaccurately, the reactor trip breakers were opened.

CORRECTIVE ACTION

The individual rod position indicator meter movement was replaced with a spare.

The indicator was retested and returned to service~

The Action Statement was terminated at 2335 hours0.027 days <br />0.649 hours <br />0.00386 weeks <br />8.884675e-4 months <br />.
