Pressurizer Pressure Channel III Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 DESCRIPTION OF OCCURRENCE:

During routine operation on July 29, 1980, the above pressure channel would not respond to changes in system pressure.

On August 1, 1980, the above pressure channel failed low.

The channel was declared inoperable at 0610 hours0.00706 days <br />0.169 hours <br />0.00101 weeks <br />2.32105e-4 months <br /> on July 29, 1980 and also at 1529 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.817845e-4 months <br /> on August 1, 1980.

Action Statement 3.3.2.1 was entered which requires that with an ESFAS instrumentation channel inoperable, operation may proceed until performance of the next required channel functional test, provided the inoperable channel is placed in the tripped condition* within one hour.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure.

ANALYSIS OF OCCURRENCE:

On both occasions the associated bistables were placed in the tripped condition within one hour.

CORRECTIVE ACTION

On July 29, 1980, the investigation revealed a shorted wire in the power supply.

The power supply was repaired, tested satisfactorily and the Action Statement terminated at 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br />.

On August 1, 1980, a comparator failed.

The comparator was replaced, tested satisfactorily and the Action Statement terminated at 1849 hours0.0214 days <br />0.514 hours <br />0.00306 weeks <br />7.035445e-4 months <br />.
