Auxiliary Alarm Typewriter Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 Prior to Initial Criticality DESCRIPTION OF OCCURRENCE:

During routine operations, it was determined that the Auxiliary Alarm Typewriter was inoperable and was.declared so at 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br />.

With the Auxiliary Alarm Typewriter inoperable, Action Statement 3.4.6.1.b was entered because the Containment Pump Level Monitoring System was no longer available.

The action required with only two of the three available systems operable is that operation may continue for up to 30 days provided g-rab samples *of the containment atmosphere are obtained and analyzed at least once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> when the required gaseous and/or particu-late Radioactivity Monitoring System is inoperable, otherwise, be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure ANALYSIS OF OCCURRENCE:

With the Auxiliary Alarm Typewriter not operating, it is impossible to obtain start and stop times of the Containment S'l.unp Pump and, therefore, the Level Monitoring System was not operable.

CORRECTIVE ACTION

An investigation revealed two problems with the Auxiliary Alarm System.

An integrated circuit chip on the memory board was not operating properly which prevented any alarm with the number 4 not to print out*

and the typewriter carriage was out of adjustment.

The chip was replaced, the*carriage adjusted by a vendor representative and the system was tested satisfactorily.

Containment air samples were taken at least every 24 hou.rs until the system was restored and the Action Statement terminated at 2310 hcurs on July 23, 1980.
