Mede 3 Prior to Initial Criticality DESCRIPTION OF OCCURRENCE:

During a routine tour of the station, it was determined that 21 and 22CS6, Spray Header Isolation Valves, were cleared and tagged in the closed position for the Shift Supervisor.

Action Statement 3.0.3 was entered when the valves were found in an abnormal position at 2236 hours0.0259 days <br />0.621 hours <br />0.0037 weeks <br />8.50798e-4 months <br /> which requires for the Unit to be placed in at least hot standby within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, in at least cold shutdown within the following 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> unless corrective measures are completed that permit operation under the permissible Action Statements for the specified time interval as measured from initial discovery.

Personnel Error ANALYSIS OF OCCURRENCE:

The valves, 21 and 22CS6, were tagged as required by a temporary

The on-the-spot change was used inappropriately.

The procedure used by Operations Department will be changed to prohibit the use of.a temporary on-the-spot change to modify any surveillance restoration step.
