Auxiliary Feedwater Storage Tank (AFWST) < 200,000 Gallons CONDTIONS PRIOR TO OCCURRENCE:

Mode 3 Prior to Initial Criticality DESCRIPTION OF OCCURRENCE:

During routine surveillance of the control console, it was determined the level in the AFWST was below the minimum level (200*, 000 gallons).

Action Statement 3.7.l.3 was entered at 0708 hours0.00819 days <br />0.197 hours <br />0.00117 weeks <br />2.69394e-4 months <br /> which requires the level to be restored within four hours or be in Hot Shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> *

Personnel Error ANALYS.IS OF OCCURRENCE:

In the process of using the AFWST as a makeup supply of water to the steam generators, the supply valve for the AFWST was throttled too far to compensate for the water removed.

"CORRECTIVE ACTION:

  • .The tank was refilled to > 200,000 gallons of stored water and the Action Statement was terminated at 0731 hours0.00846 days <br />0.203 hours <br />0.00121 weeks <br />2.781455e-4 months <br />.

The incident was discussed with the operators involved to heighten their awareness.
