Safety Valve Improperly Set CONDITIONS PRIOR TO OCCURRENCE:

Mode 4 Prior to Initial Criticality DESCRIPTION OF OCCURRENCE:

In reviewing the calibration records for the safety valves, 2PR3, 4 and 5, it was determined the valves were not set by the vendor at ambient conditions of the valve at nominal operating temperature ana pressure as required by Action Statement 3.4.3.

A discussion with the vendor ensued and it was determined the accuracy of the valves settings would be within the allowable tolerance when comparing test conditions to operating conditions.

Based on the above discussions, a mode change to Mode 3 was allowed.

As part of a routine.inspection, the Resident NRC Inspector determined the valve setting was questionable and requested the valves be retested as soon as possible.

Upon retest by Public Service Electric and Gas, it was found 2PR4 and 5 were out of specification.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Procedural Inadequacy ANALYSIS OF OCCURRENCE:

The initial review of the Technical Specification requirement did not identify the difference between Unit 1 and Unit 2 requirements.

Unit 1 does not require the testing and setting of the safety valves at ambient conditions of the valve at nominal operating temperature and pressure.

CORRECTIVE ACTION

The valves were tested and reset properly while installed in the system and the procedure has been changed to ensure the test ambient conditions are proper to comply with Technical Specifications.
