Failure of Steam Driven Auxiliary Feedwater Pump CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 Prior to Initial Criticality DESCRIPTION OF OCCURRENCE:

Upon initial heatup when performing the surveillance procedw:e, it was determined the 23 Auxiliary Feedwater Pump would not come up to speed.

The pump was declared inoperable at 0900 hours0.0104 days <br />0.25 hours <br />0.00149 weeks <br />3.4245e-4 months <br /> and Action Statement 3.7.l.2(b} was entered which requires with one Auxiliary Feedwater Pump inoperable, restore. at least three auxjliary feedwater pumps to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Apparently, personnel error.

ANALYSIS OF OCCURRENCE:

It cannot be documented but it appears during assembly of the governor control linkage, a small nut was not attached properly, rendering the valve inoperable.

CORP.ECTIVE ACTION:

An investigation revealed the valve stern on the governor valve was stuck and a nut on the linkage was off.

The governor valve was freed and the missing nut replaced.

The pump was tested satisfactorily and the Action Statement was terminated at 0055 hours6.365741e-4 days <br />0.0153 hours <br />9.093915e-5 weeks <br />2.09275e-5 months <br /> on July 14, 1980.
