Cont.ainment Air Locks Inoperable CONDITIONS PRIOR TO OCCURP.ENCE:

Mode 3, Prior to Initial Critjcality DESCRIPTION OF OCCURRENCE:

During a six day period of time with frequent entries into the Con-tainment, the air locks failed the surveillance requirement because the door seal leak rate was greater than O. 01 La when the air,rolnme between th~ door seals was pressurized to and maintained at greater than or equal to design pressure, 47.0 psig.

In each instance, Action Statement 3. 6.1. 3 was entered which r_equires with one containment air lock door inoperable:

Maintain at least the operable air lock door closed and either restore the inoperable air lock door to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or lock the operable air lock door closed.

Operation may then continue until performance of the next required overall air lock leakage test provided that the operable air lock door is verified to be locked closed at least once p~r 31 days.

Personnel error resulting in improper air lock operation *

. ~NALYSIS OF OCCURRENCE:

During the last half of 1978, similar entries into the action statement occurred on Unit 1.

The training program for controlled access areas and the requalification program were upgraded to include an explanation of the operation of the air locks.

After one year, the explanation was deleted from the requalification program.

CORRECTIVE ACTION

Upon entry into the action statement, maintenance personnel repaired the leaking seal by repositioning the gasket into its seating position and performing the applicable surveillance procedure satisfactorily.

The air lock operation presentation in the tr~ining program has been further upgraded to explain correct operation techniques.

In addition, the upgraded air lock operation presentation will be

  • added back to the requalification program.
