Vital Bus Undervoltage Relay Out of Specification CONDITIONS PRIOR TO..OCCURRENCE:

Mode 3 Prior to Initial Criticality DESCRIPTION OF* OCCURRENCE:

During performance of a routine surveillance, it was determined that the 2c;: vi.tal bus undervol tage trip relay was set at 76. 9 vol ts instead of 77.3 volts as specified.

Action Statement 3.3.2.l(a) was entered at 1115 hours0.0129 days <br />0.31 hours <br />0.00184 weeks <br />4.242575e-4 months <br />.

The channel was declared inoperable and Action 14 was ta.ken until the channel is restored to operable status with the trip

. setpoint adjus:ted consistent with the trip setpoint.

The trip relay was placed in a tripped position within one hour as required.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Change in environmental conditions.

I ANALYSIS OF OCCURRENCE:

The undervoltage relays were set to trip at the required voltage during preparation for operation.

A drastic temperature increase occurred when many pieces of equipment were energized and a plant heatup occurred causing a change in the relay setpoint.

CORRECTIVE ACTION

The undervoltage relay was reset under existing conditions and tested satisfactorily.

The Action Statement was terininated.

The ambient conditions now existing should remain relatively constant and a similar problem should not occur again.
