21 Residual Heat Removal Pump (RHR) Improperly Assembled CONDITIONS PRIOR TO OCCURRENCE:

Construction DESCRIPTION OF OCCURRENCE:

While disassembling 21 RHR Pump to install a modified shaft locking device in accordance with an approved Design Change, it was determined that at some earlier time the pump was assembled improperly.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Unknown, either personnel error or pr,ocedural inadequacy~

ANALYSIS OF OCCURRENCE:

When disassembling the pump, it was determined that the shaft sleeve was installed such that it's keyway was approximately 30° offset from the shaft's keyway.

The difference in orientation prevented the key from being installed properly after the impeller was positioned on the end of the shaft.

A review of the documentation indicated the pump impeller was last worked on during initial construction in November of 1976 but no other information was available.

CORRECTIVE ACTION

The pump was disassembled and inspected.

It was necessary, prior to reassembly, to "dress" the shaft to compensate for a small amount of galling.

The pump was reassembled corre~tly.

A modification was made to 22 RHR Pump as required under the same DCR.

No abnormalities were found when this pump was disassembled.
