Two Safety Injection Pumps available when < 312°F CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 Initial Fuel Load DESCRIPTION OF OCCURRENCE:

In pre~aration to perform a routine surveillance procedure SP(O) 4.3.2.1.3, Response Time Testing, on June 7, 1980 at 0145 hours0.00168 days <br />0.0403 hours <br />2.397487e-4 weeks <br />5.51725e-5 months <br />, both 21 and 22 Safety Injection Pumps were made available with Reactor Coolant temperature less than 312°F in violation of Technical Specification requirement 3.5.3 which limits the maximum number of safety injection pumps allowed operable to one.

The test was completed, and at 0545 hours0.00631 days <br />0.151 hours <br />9.011243e-4 weeks <br />2.073725e-4 months <br /> on June 7, 1980, the pumps were removed from service.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Procedural inadequacy.

The procedure did not state a maximum number of safety injection pumps that were allowed to be operable.

ANALYSIS OF OCCURRNECE:

Since we were never in a higher mode than Mode 5, the pre-existing procedure that provided guidelines for a routine entry into Mode 5 from Mode 4 was not consulted.

The necessary limits and actions existed in the overall Operating Instruction to ensure Technical Specification 3.5.3 was not violated.

In a review of the Operating Logs by the Resident NRC Inspector, it was determined the Technical Specification was violated.

CORRECTIVE ACTION

An on-the-spot change has been made to the Surveillance Procedure to insure no more than one safety injection pump is available when less than 312°F.
