2B Diesel Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 6 Initial Fuel Load DESCRIPTION OF OCCURRENCE:

While performing routine maintenance on 2A Emergency Diesel and performing surveillance on 2B Emergency Diesel, it was determined the 2B diesel would not increase its load to the required 2600 KW.

Action Statement 3.8.1.2 was entered at 1028 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91154e-4 months <br /> which requires with less than two diesel generators operable, suspend all operations involving core alterations or positive reactivity changes until the minimum required electrical power sources are restored to operable status.

The 2A diesel was restored to operable status and dem-onstrated operable at 1215 hours0.0141 days <br />0.338 hours <br />0.00201 weeks <br />4.623075e-4 months <br /> and the Action Statement was terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Three pins for the individual fuel injectors were stuck preventing the loading of the diesel fully.

ANALYSIS OF OCCURRENCE:

When performing the routine surveillance, 2B diesel was only able to load to 2100 KW.

When the 2B diesel was declared inoperable, work on the 2A diesel was stopped.

2A diesel was returned to operable status allowing the Action Statement to be terminated.

CORRECTIVE ACTION

The problem was investigated and it was found three pins were stuck after first replacing the governor.

The pins were made operable.

In addition, the frequency, speed and EGA controls were checked to verify operability.

The 2B diesel was returned to operable status with the surveillance being performed on June 2, 1980.

An inspection order has been issued to ensure proper lubrication of the fuel rack on a monthly basis.
