Lack of Fire Barrier CONDITIONS PRIOR TO OCCURRENCE:

Construction Phase DESCRIPTION OF OCCURRENCE:

At the_completion of construction, various pieces of equipment, electrical cables, pipes and welding cable, were removed from fire barriers and were not plugged prior to receiving our operating license and technical specifications.

In our review of plant con-ditions and technical specifications in preparation to load fuel, it was determined the penetrations were opened in violation of Technical Specification 3.7.11.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel error.

ANALYSIS OF OCCURRENCE:

The fire barriers should have been sealed prior to receipt of the license because it requires at all times all fire barrier penetrations in fire zone boundaries, protecting safety related areas, shall be functional.

CORRECTIVE ACTION

As identified, all the fire barriers were sealed prior to loading fuel.
