APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Dqcket No. 50-272 This refers to the inspection conducted by representatives of the Region I (Philadelphia) office at Salem Nuclear Generating Station, Unit No. 1, Hancocks Bridge, New Jersey of activities authorized by NRC License No. DPR-70.

During this inspection conducted on May 26 - June 30, 1979, the following apparent item of noncompliance was identified.

This item is an infraction.

Technical Specification 3.7.10.2 states that the spray/sprinkler system for the Reactor Cool ant Pump Lube-Oil System s.ha 11 be operable whenever the equipment in the spray/sprinkler protected area is required to be operable.

All four React-or Coolant Pumps are required to be in operation (and hence operable) above P-7.

With the spray/sprinkler system inope-rable:

a continuous fire watch shall be established within one hour; and the system shall be restored to an *operable status within 14 days or a Special Report shall be submitted to the NRC within 30 days.

Contrary to the above, from November 13, 1978 to Apl'il 3, 1979, with reactor power above P-7, the spray/sprinkler sy'stem for No. 13 Reactor Coolant Pump Lube Oil System was inoperable in that the header isolation valve IFP 147 was closed and the low air pressure alarm for deluge valve IFP 226 was jumpered.

The closed header isolation valve prev-ented an automatic initiation and the jumpered alarm removed the indica-tion necessary for manual initiation. Additionaliy, no fire watch was established nor was a Special Report submitted.
