APPENDIX A Public Service Electric and Gas Company NOTICE OF VIOLATION Docket No.
50~272 Based on the results of an NRC inspection conducted on April 27 - May 2 and
- May 22-23, 1979, it appears that one of*your activities was not conducted in full compliance with conditions of your NRC Facility License No. DPR-70, as.indicated below.
This item is a deficiency.
Technical Specification 6.8.1, states in part, 11Written procedures shall be established, implemented and maintained **** * **** " Administrative procedure No. 24, Rev.. 5, paragraph 6.9.3 states: "Instruments for body scans or frisking shall be a*t various locations within the plant. Personnel shall monitor themselves whenever leaving a contaminated area, and the Controlled Access Area.
11 Paragraph 6.9.4 sfates: "Individuals shall not wear Controlled Access Area protective clothi.ng in a Clean Area."
Contrary to the above, on April 30, 1979, contamination control practices appeared not to be followed when a vendor representative failed to use the frisker to *monitor himself on exiting the contamination control boundary at the containment air lock.
On May 1, 1979, the same vendor again failed to use the frisker to monjtor himself and in addition a member of the security force walked across the same stepoff pad from the contaminated side to the clean side without frisldng or removing his protective clothfog shoe covers.
