APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-311 This refers to the inspection conducted by representatives of the Reqion I (Philadelphia) office at Salem Nuclear Generatinq Station, Unit 2, Hancocks Bridge, New Jersey, of activities authorized b_y NRC License No. CPPR-53.
Durinq this inspection conducted on August 14-18, 1978. the followinq apparent item of noncompliance was identified. This item is an infraction.
10 CFR 50, Apoendix 8, Criterion V, states, in part, 11Activities affecting quality shall be prescribed bv documented instructions, orocedures,. or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, orocedures or drawi nqs.

218268-A~8903-12 prescribes a typical fillet.weld (Detail 17) for small bore piping socket connections to residual heat removal/safety-injection system piping.
It specifies a 2:1 length ratio for the fillet weld.
Contrary to the above, on August 17, 1978 the completed and accepted weld joint FW-2.RH-3378-1 did not have the required 2:1 length ratio, and the inspection criteria in the comoleted Fillet Weld History Record did not reflect the 2:1 criteria.
