Dear Mr. Grier:
NRC INSPECTION REPORT NO. 50-311/78-26 NO. 2 UNIT SALEM NUCLEAR GENERATING STATION We have reviewed the report of your inspection conducted on August 14-18, 1978, which was transmitted with your letter of September 12, 1978, and received on September 18, 1978.
Our response to the item of noncompliance is as follows:
"10 CFR 50, Appendix B, Criterion V, states, in part, tActivities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings.'
"The Salem Unit 2 FSAR Se ct ion D. 1. 6. 5 requires,
'Activities that affect quality are prescribed by PSE&G and its contractors by means of written procedures, instructions, drawings and checklists *.. '
"PSE&G Drawing No. 218268-A-8903-12 prescribes a typical fillet weld (Detail 17) for small bore piping socket connections to residual heat removal/safety-injection system piping.
It specifies a 2:1 length ratio for the fillet weld.
"Contrary to the above, on August 17, 1978 the completed and accepted weld joint FW-2.RH-3378-1 did not have the required 2:1 length ratio, and the inspection criteria in the completed Fillet Weld History Record did not reflect the 2:1 criteria."

l0/18/78 The following affirmative actions have been initiated by PSE&G:

Mechanical Division of the Engineering and Construction Department has conducted a review and an evaluation of all small bore nozzle to nipple connections in the RHR, eve

and SI systems.
It was determined that the 2Xl unequal leg fillet weld is applicable for one hundred and seventy-one (171) field welds in these systems.
UE&C Quality Control has performed a visual inspection of the respective field welds and found that the 2Xl requirement was not accomplished.
Accordingly, Deficiency Report (DR)
- 6513 was initiated and forwarded to PSE&G Engineering for review and disposition.
The Engineering disposition specified that the additional welding be performed to achieve the required weld size followed by a liquid penetrant examination and pressure test.
All work has been completed in accordance with the disposition, with the exception of pressure testing.
Pressure testing of the subject welds will be accomplished during Hot Functional Testing (HFT).
Substantiation of Engineering position will be available on site prior to HFT.
In addition, the UE&C Quality Control Department performed a review of all Safety Related and Nuclear Class I, II, and III mechanical arrangement drawings to determine that applicable field welds are in accordance with the special weld details which are specified on these drawings.
As a result of this review, it was determined that there were six additional welds which were not in accordance with special weld details.
Deficiency Reports 6806, S-0038, and S-0039 have been issued.
The Engineering disposition of these DR's will require that additional welding be performed on these welds to achieve conformance with the applicable special welding detail, appropriate nondestructive examination be performed after welding, followed by pressure test.
To preclude future items of noncompliance, the UE&C Quality Control and Welding Departments have issued letters of instruction to their personnel who prepare and review weld history records.
These letters require that a thorough review of mechanical arrangement drawings for special weld details be performed prior to the preparation and issuance of weld history records.
