On April 16, 1997, the NRC completed an inspection at your Palisadl?ls Nuclear Generating
Plant. The enclosed report presents the results of that inspection.
The inspection covered a 6-week period. Areas examined during the inspection are
identified in the report. Within these areas, the inspection consisted of a selective
examination of procedures and representative records, interviews with personnel, and
observation of activities in progress. The purpose of the inspection effort was to
determine whether activities authorized by the license were conducted safely and in
accordance with NRC requirements. The summary contained in the report provides an
overview of our assessment of your performance.

Based on the results of this inspection, the NRC has determined that violations of N~C
requirements occurred. The violations are cited in the enclosed Notice of Violation
(Notice), and the i;ircumstances surrounding them are described in detail in the enclosed
report. The first violation is of concern because it resulted in a Code violation as a result
of unauthorized Code repairs performed to the main steani isolation valve stuffing box
plugs. The second violation is of concern because of your staff's failure to properly submit
a 10 CFR 50. 73 report within the time frame. required. The third violation involves three
examples of a failure to follow procedure associated with the repair of pipe plugs for the
main steam isolation valves. These violations represent poor management and contractor
oversight of safety related system maintenance activities.
You are required to respond to this letter and should follow the instructions specified in the
enclose~ Notice when preparing your response. In your response, you should document
the specific actions taken and any additional actions you plan to prevent recurrence. Your
response may reference or include previous docketed correspondence, if the
correspondence adequately addresses the required response. After r~viewing your
response to this Notice, including your proposed corrective actions and the results of
future inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements .
