SUBJECT:
NRC INSPECTION REPORT NO. 50-255/97011(DRP) AND NOTICE OF
VIOLATION
Dear Mr. Palmisano:
On October 17, 1997, the NRC completed an inspection at your Palisades Nuclear Generating
Plant. The enclosed report presents the results of that inspection.
The inspection covered a 7-week period. Areas examined during the inspection are identified in
the report. Within these areas, the inspection consisted of a selective examination of procedures
and representative records, interviews with personnel, and observation of activities in progress.
The purpose of the inspection effort was to determine whether activities authorized by the license
were conducted safely and in accordance with NRC requirements. The summary contained in
the report provides an overview of our assessment of your performance.
During this inspection period the licensee performed, after the issues were brought to their
attention, prompt and thorough reviews of increased turbine vibration, thermal margin monitor
failures and a lack of availability of emergency diesel generator lube oil. However, opportunities
existed to identify and address these issues in a more timely manner. System engineers review
of the high pressure safety injection system and high pressure air system were positive examples
of critical reviews.
Based on the results of this inspection, the NRC has determined that one violation of NRC
requirements occurred. The violatiqn is cited in the enclosed Notice of Violation (Notice), and the
circumstances surrounding it are described in detail in the enclosed report. This violation was of
concern because the loss of equipment control due to an inappropriate maintenance procedure
resulted in venting the waste gas surge tank to the auxiliary building atmosphere, resulting in the
ci:>ntatnination of the maintenance crew.
You are required to respond to this letter and should follow the instructions specified in the

- detennine whether further NRC enforcement action is necessary to ensure compliance with NRC
regulatory requirements .
