PART 2 GENERIC LETTER 89-10 INSPECTION
{NRC INSPECTION REPORT NO. 50-255/94015{DRS})
Dear Mr. Palmisano:
This letter refers to the routine safety inspection conducted by
Mr. Marc P. Huber and others of this office on September 12 through 30, 1994.
The inspection included a review of activities authorized for your Palisades
Nuclear Generating Plarit.
At the conclusion of the inspection, theJfindings
were discussed with you and members of your staff identified in the enclosed
report.
The areas examined during the inspection consisted primarily of a review of
the Palisades Motor Operated Valve {MOV) Program established in response to
Generic Letter 89-10. Within this area, the inspection consisted of a
selective examination of procedures and representative records, observations,
and interviews with your personnel.
The purpose of the inspection was to
determine whether activities authorized by the license were conducted safely
and in accordance with NRC requirements.
The inspection's results iridicated th~t Palisades Plant management provi~ed
ineffective direction and oversight of the Generic Letter 89-10 motor ~perated
valve {MOV) program.
This resulted in the MOV program implementation showing
virtually no significant progress for the past two years; almost no design
basis testing was performed, engineering calculations contained numerous
technical problems, and numerous corrective actions for potential MOV
deficiencies were incomplete and untimely.
We understand that corrective
actions will be taken to resolve the noted deficiencies.
Further, we
understand that all planned design basis MOV testing will be completed during
the Palisades' spring 1995 refueling outage.
We expect, within one month
after restart, that the design-basis capability of all program MOVs will have
been verified and that programmatic assumptions will have been validated.
Our inspection report is being provided to NRR as input for.evaluation of your
request for a schedule extension.
Formal response to your request will be
addressed by NRR.
Based on the inspection's results, certain of your activities appeared to be
in violation of NRC requirements, as specified in the enclosed Notice of
Violation {Notice).
The violation is of concern because MOV operability was
in question and appropriate action was not taken to resolve the issues .

specified in the enclosed Notice when preparing your response. * In your
response, you should document the specific actions taken and any additional
actions you p 1 an to prevent recurrence.

plan for in-sftu design-basis testing and the commitment to complete that
testing prior to startup. *After r~viewing your responie to this Notice,
including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure conipliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of
. this letter.and its enclosures will be placed in the NRC Public Document Room.
The *responses directed by this 1 etter and the enclosed Notice are not subject
_ to the clearance procedures of the Office of Management and Budget as required
. by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
