SUBJECT:
NRC INSPECTION REPORT.NO. 50-255/94004
This refers to the routine safety inspection conducted by Messrs.
M. E. Parker, D. G. Passehl, K. Salehi, and J. H. Neisler of this office from
January 29, 1994, through March 14, 1994.
The inspection included a review of
activities at your Palisades Nuclear Generating Facility.
At the conclusion
of the inspection, the findings were disc~ssed with those members of your
staff identified in the enclosed report.
Areas examined during the inspection are identified in the repo~t. Within
these areas, the inspection consisted of a selective exami~ation of procedures
and representative records, ob~ervations, and interviews with personnel.
While no violatirins of NRC requirements were identified during the course of
this inspection, an unresolved item was identified conc~rning missing
separation barriers for Reactor Protection System (RPS) channels 1 a.nd 3
cables located in the same cable tray. Although your initial operability
determination found the RPS channels to be operable, your subse~uent review at
our request found that the RPS channels may not have b~en operable as
. required. This indicates that weaknesses in engineering.reviews of system
design requirements still exist, despite your efforts to improve in this area.
In addition, this inspection identified some long-standing material condition
deficiencies that were not corrected in a timely manner, including the "A" air
start motor for Emergency Diesel Generator (EDG) 1-2, and a problem
annunciator associated with lube oil level switch LS 1487 on EDG 1-2.
We will
continue to evaluate y6ur actions in regard to these issues.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosed inspection report will be placed in the NRC*
Public Document Room.
