SUBJECT:

NRC INSPECTION REPORT NO 50-255/91017(DRP)

This refers to the special safety inspection conducted by Messrs. E. R.

Schweibinz and J. R. Roton of this office on September 10 through 20, 1991.

The inspection included a review of activities authorized for your Palisades Nuclear Generating plant.

At the conclusion of the inspection, the findings were discussed with you and those members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report.

Based on the results of this inspection, three apparent violations were identified and are being considered for escalated enforcement action in accordance with the 11General Statement of Policy and Procedure for NRC Enforcement Actions 11 (Enforcement Policy), 10 CFR Part 2, Appendix C (1991).

The apparent violations involved the failure to have adequate procedures for the return of containment spray pumps to service as required by Technical Specification (TS) 6.8.1.a, the failure to have containment spray pump P-54C operable when the reactor was made critical on March 10 and 26, 1991, as required by TS 3.4.1, and the failure to perform actions required by TS 3.4.2 during plant power operations between March 10 and May 23, 1991, while containment spray pump P-54C was inoperable.

No Notice of Violation is presently being issued for these inspection findings.

In addition, please be advised that the number and characterization of apparent violations described in the enclosed inspection report may change as a result of further NRC review.

An enforcement conference to discuss these apparent violations has been scheduled for October 9, 1991.

The purposes of this conference will be:

to discuss the apparent violations, their causes and safety significance; to provide you the opportunity to point out any errors in our inspection report; to provide an opportunity for you to present your proposed corrective actions; and to discuss any other information that will help us determine the appropriate enforcement 9110080017 911001 PDR ADOCK 05000255 G

action in accordance with the Enforcement Policy.

In particular, we expect you to address: any bases for considering containment spray pump P-54C operable in

  • the subject period; the safety implications of an inoperable P-54C alone, and occasions and duration of other equipment inoperability in the subject period; and your maintenance program for fuse holders, including procedures, schedules, and bases.

You will be advised by separate correspondence of the results of our deliberations on this matter.

No response regarding the apparent violations is required at this time.

In accordance with 10 CFR 2.790 of the NRC's 11 Rules of Practice, 11 a copy of this letter and its enclosure will be placed in the NRC Public Document Room.
