This refers to the routine safety inspection conducted by Messrs. E. R. Swanson,
N. R. Williamsen and T. V. Wambach during the period of March 3 through April 4,
1988, of activities at the Palisades Nuclear Generating Plant authorized by
NRC Provisional Operating License No. DPR-20 and to the discussion of our
findings with Mr. D. P. Hoffman and others of your staff at the.conclusion
of the inspection.
The enclosed copy of our inspection report identifies areas examined during
the inspection.
Within these areas, the inspection consisted of a selective
examination of procedures and representative records, observations, and
interviews with personnel.
During this inspection, certain of your activities appeared to be in violation
of NRC requirements, as specified in the enclosed Notice.
A written respons~
is required.
With respect to item 1 it is noted that recent review of this
issue resulted in further failure to identify the unreviewed safety question
due to misleading changes made to your Updated Safety Analysis Report.
Please
outline any actions you plan to take in this regard.
With respect to item 2
it was noted that the personnel involved appeared not to be proficient in the
performance of the test procedure.
Please include in your reply any actions
you are taking to assure correct performance of procedures.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosures, and your response to this letter will be
placed in the NRC Public Document Room.
The responses directed by this letter
and the accompanying Notice are not subject to the clearance procedures of the
Office of Management and Budget as required by the Paperwork Reduction Act of
1980, PL-67-511.
