NOTICE OF VIOLATION Consumers Power Company Docket No. 50-255 As a result of the inspection conducted on March 3 through April 6, 1987 and in accordance with the General Policy and Procedures for NRC Enforcement Actions (10 CFR Part 2, Appendix C), the following violation was identified.
10 CFR 50 Appendix B Criterion XVI "Corrective Action" requires in part that 11 In the case of significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition." Palisades Administrative Procedure No. 3.03 "Corrective Action 11 implements this requirement to take corrective action to preclude repetition in Articles 4.7, 6.3, 6.4, 6.5 and 6.6.
Contrary to the above on November 8, 1985, a Low Pressure Safety Injection Pump and a Safety Injection Tank were concurrently inoperable and the corrective actions described in the licensee's January 22, 1986, and April 11, 1986 letters were not adequate to preclude repetition. A second similar event involving a concurrently inoperable Diesel Generator and Component Cooling Water pump took place on March 10, 1987.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of the Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results *achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
