Appendix A NOTICE OF VIOLATION Mr. Anthony S. Kanicki, Jr.
License No. SOP-3160-1 Docket No. 55-4007 Based on the results of an NRC inspection conducted August 19-20, 1980, it appears that one of your activities was not conducted in full compliance with cond1tions of your Senior Operator's License as indicated below.
This is an infraction.
Senior Operator's License No. SOP-3160-1 states in part that, "In directing the licensed activities of licensed operators and in manipulating the con-trols of the Palisades facility the licensee shall observe the operating procedures and other conditions specified in the facility license which authorizes operations of the facility... "
Appendix A to Provisional Operating License DPR-20, Technical Specification Section 6.8.1 requires written procedures for surveillance and testing activities on safety-related equipment to be implemented.
Safety-related surveillance procedure Q0-2, Recirculation Actuation System, has as a prerequisite that the plant be in cold or refueling shutdown when the test is performed.
Contrary to the above, on August 19, 1980, the licensee authorized the implementation of surveillance test Q0-2, involving the stroking of valve CV-3031, while the plant was in power operation.
