Dear Mr. Kanicki:
SEP 1 6 1980 This refers to the special inspection conducted by Messrs. B. A. Jorgensen and J. Heller of this office on August 19-20, 1980, concerning a safety-related equipment surveillance test, Q0-2.
Based on the result of this inspection it appears that one of your activities was not conducted in full compliance with the requirements of your Senior Operator License No. SOP-3160-1 as set forth in the Notice of Violation, enclosed herewith as Appendix A.
This 'item of noncompliance has been categorized into the levels as described in the attached enclosure dated December 31, 1974.
This notice is sent to you pursuant to the provisions of Section 2.201 of the NRC's "Rules of Practice,"
Part 2, Title 10, Code of Federal Regulations.
Section 2.201 requires you to submit to this office within twenty (20) days of your receipt of this notice, a written statement in reply including admission or denial of the item of noncompliance and if admitted, the reasons for the item of noncompliance.
Enforcement action is also being taken against your employer, Consumers Power Company.
Copies of the correspondence relating to these actions are available in the Public Document Room.
Copies are also enclosed for your information.
A copy of this letter and the enclosed Notice of Violation is being transmitted to your employer, Consumers Power Company, holder of NRC License No. DPR-20.
In accordance with Section 2.790 of the NRC "Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and your reply will be placed in the NRC's Public Document Room.
