REPORT (50-220/89-200

AND 50-410/89-200)

This letter refers to the

NRR Special

Team Inspection led by Anthony T. Body of

this office on January

31-March 3, 1989, at Nine Mile Point Units

1 and 2,

Scriba,

New York.

The results of the inspection are documented

in the enclosed

inspection report.

At the conclusion of the inspection,

an exit interview was

held with you and members of your staff to discuss

the scope

and the findings

of the inspection.

The purpose of this inspection

was to perform an independent,

in-depth

assessment

of the effectiveness

of management

oversight, including critical

self-assessment,

of the operational

safety performance of the facility.

The

inspection

team performed

an integrated evaluation of various technical

areas,

including operations,

maintenance,

surveillance, training, engineering,

and

safety assessment.

Within these areas,

the inspection consisted of interviews

with personnel,

observation of plant activities,

and selective

examinations of

procedures,

records,

and documents

by the inspectors.

Several significant concerns

were identified during the course of the

inspection that warrant your prompt attention.

Specifically, your corrective

action programs,

procedure controls, relationships

between Unit

1 operators

and

training personnel,

and your manageItIent oversight of plant activities were

found to be deficient.

The inspection also identified that the standards

of procedural

adherence

were not clearly understood

and followed by personnel

at both Units

1 and 2.

Your staff agreed to revise

and clarify, as necessary,

guidance to plant

personnel

on the use of procedures

and provide additional training on the

guidance prior to restart of Unit 2.

The completion of the actions

was

documented

in your letter to the

NRC on Yarch 30,

1989,

Corrective Actions have

been developed in the Nuclear

Improvement Program

and

Unit

1 Restart Action Plan in an attempt to resolve the many significant prob-

lems.

However, Niagara

Mohawk has

a history of proposing

improvements to satisfy

NRC concerns

and not effectively implementing the changes.

Significant cultural

changes

are required throughout your organization to ensure
