As a result of the inspection conducted on April 1, 1981 to May 1, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1981), the following violations were identified:
A.
Technical Specification 6.8.1 states:
"Written procedures and administrative policies shall be established, implemented and maintained that meet or exceed the requirements and recommendations of Sections 5.1 and 5.3 of ANSI N18.7-1972 and Appendix "A" of USAEC Regulatory Guide 1.33 except as provided in 6.8.2 and 6.8.3 below."
Regulatory Guide 1.33, Revision 0, dated November 1972 states in Appendix A:
"The following are typical safety-related activities that should be covered by written procedures."
In Section G. 1.C of Appendix A it lists:
"Liquid Radioactive Haste System-Discharging to Effluents."
Contrary to the above, on April 14, 1981, 811 Waste Sample Tank was discharged to Lake Ontario and a written procedure had not been established for this evolution.
This is a Severity Level V Violation (Supplement I).
B.
Environmental Technical Specification 5.5.1 states, in part:
"Detailed written procedures... shall be prepared, approved as specified in Section 5.5.2, and adhered to for operation of all systems...
Procedures shall include sampling, instrument calibration,...
Calibration frequencies for instruments used in performing the measurements required by the environmental technical specifications shall be included.
Those frequencies shall be determined from experience with similar instruments in similar environments and from manufacturer's technical manuals."
Contrary to the above, on April 14, 1981, the licensee had failed to establish a calibration frequency for the liquid waste discharge flow
- recorder, 8IM22, and used this flow recorder while discharging 811 Waste Sample Tank to Lake Ontario.
This is a Severity Level VI Violation (Supplement I).
