I'NSPECTION

REPORT

NOS. 50-259/89-Ã,

50-260/89-16,

AND 50-296/89-16

This refers to the Nuclear Regulatory

Commission

(NRC) team inspection

led by

M. Branch during the weeks of November 27,

1989 and December ll, 1989.

The

inspection

included

a review of activities associated

with the preparation

for

startup of Browns Ferry Unit 2.

At the conclusion of the inspection,

the

findings were discussed

on December

20,

1989 with those

members of your staff

identified in the enclosed

inspection report.

The purpose of this in'spection

was to evaluate

the effectiveness

of the

corrective actions that have

been

conducted

to support Unit 2 'startup.

The

implementation of selected

portions of the

TVA Nuclear Performance

Plan

Volume

3 Corrective Action Programs that pertained

to the core spray system

was

evaluated.

The results of this inspection will be used to evaluate

the need

for additional

NRC activity in specifically identified areas.

This inspection

was

composed of design

and as-constructed

aspects.

Contact

was

maintained

between

inspectors

in ea'ch area

so that their respective efforts and

inspection findings were coordinated.

The design portions consisted of a.

selective

review of the core spray system design,

design bases,

and design

requirements.

The as-constructed

portion consisted of an in-plant review in

the mechanical, electrical, civil, structural,

and instrumentation

and control

disciplines in order to verify that the core spray system

as currently construc-

ted and installed is in accordance

with the licensed design basis

and applic-

able drawings.

In addition, the operational capability of the core spray

system

was evaluated

by reviewing the system operating instructions

and proce-

dures,

surveillance

and testing requirements,

corrective

and preventive main-

tenance activities,

and operator training.

The inspection

team evaluated,

on

a sampling basis,

portions of the

TVA Nuclear Performance

Plan functional

corrective action areas.

The

NRC team concluded that the core spray system

was functional, subject to

the scheduled

TVA modifications,

and that 'the design

was adequately

supported

by calculations.

Additionally, on the basis of the review of the core spray

system,

TVA appears

to be adequately

implementing your

NPP Volume

3 programs in

the following areas

to support Unit 2 startup.

In order to apply the results of this inspection to other systems

at Browns

Ferry,

TVA must ensure that the level of program completion

and system review

are not reduced.

The inspection findings indicate that certain activities appeared

to violate

NRC requirements.

The identified violations involved (I) ineffective correc-

tive action in the areas of calcu'1ations,

welding records,

and identification

of hardware deficiencies;

(2) failure to comply with drawing requirements

during initial hardware installation;

(3) failure to comply with plant proce-

dures;

and (4) inadequate

plant procedures.

The violations, references

to

pertinent requirements,

and elements

to be included in your response

are

described

in the enclosed

Notice of Violation.

As discussed

during the exit, and during

a subsequent

phone conversation with

the Browns Ferry Site Director, the corrective actions for some of the viola-

tions should specifically address

the following areas:

(I)

Ensure that the

SPAE and

SPOC process,

'which is essential

in ensuring that

systems

are ready for operations,

continues

and that the walkdown portions

of the process

are strengthened.

(2)

Bound the scope of calculation problems identified by the team and ensure

they are resolved

on

a generic basis for all systems

covered

by your

Design Baseline

and Verification Program

(DBVP) calculation program.

(3)

Additionally, several

hardware-related

items were identified and

TVA

indicated that resolution of these

items would be deferred to resolution

of Unresolved Safety Issue

(USI) A-46, "Seismic gualification of Equipment

in Operating Plants."

The

NRC staff position on resolution of these

hardware

items

was discussed with your staff during the course of the

inspection

and again during the exit meeting.

Your response

should

address'ow

you plan to comply with the staff's position to ensure that

non-conforming hardware-related

items identified during your system

walkdowns are properly evaluated

and resolved.

Your response

should also specify

how you plan to address

the generic applica-

bility of the team's findings to other systems.
