ENCLOSURE 1
NOTICE OF VIOLATION Tennessee Valley Authority Browns Ferry 1, 2, and 3
Docket Nos.
50-259, 50-260, and 50-296 License Nos.
DPR-33, DPR-52, and DPR-68 During the Nuclear Regulatory Commission (NRC) inspection conducted on July 15 - August 15,
- 1989, two violations of NRC requirements were identified.
The violations involved failure to comply with procedures during the performance and review of surveillance instructions, and failure to initiate compensatory measures for open fire doors.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (1989), the violations are listed below:
A.
TS 6.8. 1. lc requires that written procedures be established, implemented and maintained covering surveillance and test activities for safety related"equipment.
Site Directors Standard Practice (SDSP)
- 2. 1, "Site Procedures and Instructions,"
requires that the site be operated and maintained in accordance with written, approved procedures and instructions which have been formally issued and distributed for use.
Contrary to the
- above, on July 14,
- 1989, during the performance of procedure 3-SI-4.5.C. 1(2),
EFCW Pump Operation Surveillance Instruction for the "D1" emergency equipment cooling water (EECW) pump and during the subsequent SI review process, procedures were not properly implemented as follows:
1.
Complete vibration data as required by step
- 7. 13.24 was not taken and the vibration acceptance criteria (step
- 7. 13.25) were signed off as being within the specified limits.
2.
A complete analysis of the SI data was not performed within four working days of the test as required by step 3.7.
,Test data review was not completed until six days following the test.
The above instances are examples of a
Severity Level V Violation (Supplement I) applicable to all three units.
B.
TS
- 3. 11.G. 1.a requires that compensatory measures be established within one hour when fire rated assemblies are rendered inoperable.
Fire Protection Plan procedure FPP-2, Revision 3, Fire Protection-Attach-
- ments, implements inpart TS
- 3. 11.G.1.a and requires the preparation and processing of Attachment F,
Fire Protec'tion Equipment and Barrier Penetration Removal From Service Permit prior to impairment of the fire rated assembly.
Contrary to the
- above, on August 8,
- 1989, the NRC inspector, while on tour, identified two fire rated doors, numbers 607 and 455 wedged open.

DPR-33, DPR-52,DPR-68 Neither door had an Attachment F in effect and the SOS was not aware of the condition.
Neither door had compensatory measures in effect.
The above instances are examples of a
Severity Level IV Violation (Supplement I) applicable'o all three units.
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit a written statement or explanation to the Nuclear, Regulatory Commission, ATTN:

- 20555, with a
copy to the Associate Director for Special
- Projects, Office of Nuclear Reactor Regulation and a copy to the NRC Resident Inspector, Browns Ferry, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a
"Reply to a Notice of Violation" and should include:
(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results
- achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is
- shown, consideration will be given to extending the response time.
