The

following violation

was

identified

during

an

inspection

conducted

on

July 27 - August 19,

1985.

The Severity

Level

was

assigned

in accordance

with

the

NRC Enforcement Policy ( 10 CFR Part 2, Appendix C)

Technical Specification 6.3.A. 1

requires that detailed written procedures,

including

applicable

checkoff lists,

shall

be

prepared,

approved,

and

adhered

to for normal

startup,

operation

and

shutdown

of all

systems

and

components

involving nuclear safety of the facility.

Contrary to the

above,

this requirement

was

not met for the following two

examples:

a.

Operating

Instruction

OI-65,

Standby

Gas

Treatment

(SBGT)

System

Operation

did not

address

that

the

charcoal

bed

heaters

had to

be

manually reset after the

SBGT sy'tem

was

secured.

Although Licensee

Event

Report

259/85-29

implies this

was

not previously

known,

Final

Safety

Analysis

Report

Figure

5.3-9

note

six describes

the

manual

reset.

b.

On July 30,

1985, Surveillance Instruction

SI 4.7. B-l,

SBGT Operability

Test was being

used which did not contain

a change

dated July 7,

1985,

that

required

the

charcoal

bed

heaters

to

be

reset

after

system

shutdown.

A review of completed

SI 4.7.B-1 procedures

since

the change

was

implemented

found three

out of

10 did not contain

the

change.

Operations

personnel

are instructed

by Operations

Section

Instruction

Letter

SIL-40

to

compare

page-by-page

procedures

used

against

a

controlled copy of the procedure

to insure revisions

are incorporated.

This is

a Severity

Level

IV Violation (Supplement I) applicable to all three

units.

Pursuant

to

10

CFR %.201,

you are

required

to submit to this office within 30

days of the date of this Notice,

a written statement

or explanation

in reply,

including:

( 1) admission

or denial

of the alleged violations;

(2) the

reasons

for the violations if admitted;

(3) the corrective

steps

which

have

been

taken

and

the results, achieved;

(4) corrective

steps

which will be

taken

to avoid

further violations;

and (5) the date

when full compliance will be achieved.
