SUMMARY

An Inservice Inspection (ISI) of selected class 1, 2, and 3 components at the Shearon Harris Nuclear Power Plant, Unit One (1), was conducted during the March May 1991, RFO3 refueling outage. These'examinations constituted the first collection of ISI data in the second inspection period of the first inspection interval.

The components were selected from the Shearon Harris Ten Year Inservice Examination Plan. Manual and remote manual examination techniques were utilized. Representative samples of components selected for examination consisted of vessels and their support components, piping systems and welds, component supports, flange bolting and valve bolting.

All nondestructive ezaminations, flaw evaluations, and supporting documentation meet the requirements of ASME Section XI, 1983 Edition up to and including the summer 1983 Addenda. Manual nondestructive examinations were performed using Magnetic Particle (MT), .Liquid Penetrant (PT), Ultrasonic (UT), and Visual (VT) methods. A visual examination of the accessible areas of the Reactor Pressure Vessel interior was performed using a remote controlled mini-sub, with examination results recorded on data sheets. Also, the examination results were stored on magnetic video tapes, which contain ezamination scanning information as described on the data records.

All examination records contain the following information and are in compliance with IWA-6000 of ASHE Section XI:

ISI isometrics for components examined are listed in the appropriate ISI summaries and applicable drawings are included in section 11. Permanent component identification reference points were established and marked on the component in accordance with ASHE Section XI> Article XII-4000 during PSI, Calibration standards utilized in the performance of ultrasonic system calibration and examination were supplied by CPEcL and the NDE contractors. A list of calibration standards is found in section 6.

Where component configuration, adjacent part, or physical restriction prevented coverage of the entire required examination volume, the condition was identified and documented on the affected examination record. Section 9 contains a list of relief request that cover these examinations. All nondestructive examinations, which revealed relevant indications or conditions were identified and documented on the affected examination record. Relevant indications or conditions exceeding the allowable limits specified in the NDE procedures were evaluated in accordance with the provisions set forth in ASHE Section XI, IWA-3000.

In'he process of performing ISI examinations of the Pressurizer Integral Attachments, rejectable linear indications were noted on the ends of Integral Attachments SWB-A1B and A2A in accordance with NES procedure 83A0284 Rev.2 Addenda 002. Appendix A, Paragraph G (2) of the procedure and ASHE Section XI, Paragraph IWB-3200, Supplemental Ex'aminations, provide for other examination methods to evaluate the indications. The attachments were welded to a pressure retaining vessel greater than 2" thickness (Pressurizer) therefore, ASHE Section V, Paragraph T-441.9 and NES Procedure 83A0288 were utilized to record the extent of laminar reflectors in the integral attachments. Final evaluation of the reflectors was in accordance with T-451.3.2.

Since there was no total loss of back reflection 'with an accompanied continuous indication (para.T-441.9.2), the recorded reflectors did not require evaluation to ASME Section XI IWB-3516.2 and Table IWB-3511-2. They are therefore considered acceptable.

Repair and Replacement activities conducted from the end of the second refueling outage through the third refueling outage are contained in section 13. This section consists of PCR (Plant Change Request) or WRhA (Work Request &. Authorization) document number, description of the activity, and NIS-2 Forms.

EDDY CURRENT EXAMINATION CAROLINA POWER h.:LIGHT SHEAROH HARRIS NUCLEAR-POWER PLANT UNIT 81 STEAM GENERATORS A, 8 5. C MARCH - APRIL 1990

SUMMARY

OF RESULTS An eddy current examination was performed at the Shearon Harris Nuclear Plant, Unit ~1 in March and April 1991 on Steam Generators "A", "8" and "C". The examination was performed using the MIZ-18A .digital acquisition system.

The steam generators were examined in parallel from the inlet side using SH-10 remote fixtures. The examination was performed full length on 100% of the accessible tubing with a 0.610" probe diameter with the exception of row I and

2. Rows I and 2 were examined through the U-bend to the top cold leg support (11C) with a 0'.590" probe diameter. lhere were a few tubes that were restricted with the normal probe and were subsequently re-examined with a smaller probe.

Shot peening was also being performed at the same time as the eddy current examination and several tubes could not be examined through the cold leg tube end. This was due to the cold leg fixture's mounting foot obstructing the tube ends. These tubes can be identified with an extent tested of TEH-TSC.

The data was evaluated from digital magneto-optical discs after the data was transferred from the digital magnetic tapes. Zetec's EDDYNET software Version 2G was used for the analysis. All data was evaluated in accordance with Shearon Harris's Data Analysis Guideline Revision 0.

Steam Generator "A" There were 4314 tubes examined full length, 25 tubes examined full length with the exception of'he remaining few inches due, to the cold leg fixture obstruction and 227 tubes examined through the U-bend to llC (rows I and 2).

Three tubes were plugged in steam generator "A" for indications in the hot leg tube sheets. These were Row 23 Column 99, Row 20 Column 99 and Row 6 Column

94. These indications are indicative of primary side stress corrosion cracking (PWSCC) that have been found in other steam generators of this type.

CAROLINA POWER & LIGHT Shearon Harris Plant Steam Generators March h. April 1991 There were three tubes reported with less than. 20 percent through wall wear at the anti-vibration bars. These were all located in the T-slot area. One tube, Row 31 Column 64, had a 21 percent through wall indication at AV2.

Row 47 Column 56, had a less than 20 percent through wall pre-heater wear scar indication at the second cold leg support (02C) . This indication was previously reported as a 39 percent through wall indication. The difference in the evaluation results is due to the use of a pre-heater wear scar standard that simulates the geometry of this type of flaw.

There were three tubes with free span indications reported above 01C. Row 6 Column 58 had a 31 percent through wall indication reported, Row 3 Column 62 had a 23 percent through wall indication reported and Row 8 Column 88 had a less than 20 percent through wall indication reported., The baseline data was reviewed on these tubes and the indications were present. Row 6 Column 58 was examined with a motorized rotating pancake probe (MRPC) ahd no reportable indications were observed.

A number of tubes were reported with dis'torted roll indications (DRIs) and were later examined with an MRPC probe. Tubes that had no discontinuity indications observed on the MRPC examinagion were changed to distorted roll signals (DRS).

Steam Generator "8" There'were 4337 tubes examined full length, 9 tubes examined full length with the exception of the remaining few inches due 'to the cold" leg fixture obstruction and 228 tubes examined through the U-bend to 11C (rows I and 2).

There were no discontinuity indications observed that were greater than or equal to 40 percent through wall.

Four tubes were observed with greater than 20 percent through wall indications at the anti-vibration bars. Three of the four tubes were located in the T-slot area.

There was one tube, Row 4 Column 43, with a less than 20 percent free span indication reported above OIH.

A number of tubes were reported with distorted roll indications (DRIs) and were later examined with an MRPC probe. There were no 'discontinuity indications observed on the MRPC examination and the calls were changed to DRS.

April 1991 Steam Generator "C" There were 4320 tubes examined full length, 20 tubes examined full length with exception of the remaining few inches due to the cold leg fixture 'he obstruction and 228 tubes examined through the U-bend to 11C (rows 1 and 2).

There was one tube, Row 29 Column 45, plugged for an AVB indication of 41 percent through wall. There were several tubes in the same area of the steam generator with AVB indications reported. A total of 19 tubes were reported with AVB indications.

Two tubes were observed with free span indications above the second co1d'eg support (02C). These indications were present on the baseline data.

A number of tubes were reported with distorted roll .indications (DRIs) and were later examined with an MRPC probe. There were no discontinuity indications observed on the MRPC examination and the calls were changed to ORS.

In the appendices of this report are computer printouts with the result of the examination and .various tube sheet maps. '.Included in the complete examination results are the results of the October 1989 results for comparison.

The master copy of the digital tape cartridges were left at the plant site for future refer ence. The Master copy of the magneto-optical discs are at the Conam lab and will be kept on file.

Program was performed to ASME Section XI, 1983 Edition thru Summer 1983 Addenda, The examiners met the requirements to perform VT-3 and VT-4 examinations as prescribed in IWA-2000 Examination and Inspection requirements.

As required by IWB, IWC, IWD, and IWF subsections, component support examination quantities were based on Inspection Program B Tables (min/max) requirements.

Based on the criteria referenced above, of the 2443 total hanger population identified in the ISI program, 399 (16.3/) of the pipe supports were visually examined during this refueling outage (RFO-3). Twelve (12) component supports and thirty-seven (37) welded pipe attachments were examined and found to be satis-factory.

As a result of the designated VT-3/4 examinations, the follows.ng conditions were noted:

a. Twenty (20) hangers were identified with degraded cotter pins at the load pins. This condition did not impair the function of the hangers. New cotter pins wer'e installed.

b. Eight (8) 'hangers were re-al'igned as a result of exam-ination. All cases of misalignment identified were corrected, including the examples that were within design tolerance.

c. One (1) hanger had spherical bearing displacement at the steel attachment end that required repositioning and staking as recommended by engineering. Hanger function was not affected.

Eleven (11) other cases of spherical bearing displacement were noted. These were determined by engineering to be within design specification based on the fact that over-sized washers were installed and functioning to 'prevent bearing disengagement.

d. One (1) hanger was observed with a beveled washer installed backwards. The washer did not impair the function of the hanger, but the orientation was corrected.

e. Two (2) hangers were identified with locknuts on the struts that could be turned by hand. The dimensions of the struts were unchanged and the function of the hangers was not impacted.

'The nuts were tightened.

f ~ One (1) mechanical snubber was identified with a

covering of.a red colored debris. The snubber was removed. and function tested in the as found condition with satisfactory results.

g. One (1) mechanical snubber was identified with a small dent on the outer tube. The snubber was removed, tested in the as found condition, and reinstalled. The test results were satisfactory.
