Attachment to CPRL Letter of Response to NRC Report RH: 3RH 50-000/85-11-01 Re rted Violation:

10 CFR 50, Appendix B, Criterion V, as implemented by Carolina Power and Light FSAR, Section 1.8.5.5., requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures or drawings..., and shall be accomplished in accordance with these instructions, procedures or drawings.

Drawing numbers CAR-2167-G-2000 and CAR-2167-C-056 specify the location for reinforcing steel in Beam 55-L-KZ in the fuel handling building (pour number 1FHSL286-032) and Beam 38 in slab placement IRASL 216-009 in the reactor auxiliary building.

Contrary to the location requirements specified on these drawings, the top reinforcing steel in Beam 55-L-KZ in the fuel handling building (pour number 1FHSL286-032) and the bottom reinforcing steel in Beam 38 in pour number IRASL 216-009 in the reactor auxiliary building are one-half to one inch beyond specified tolerances.

This is a Severity Level IV Violation (Supplement II).

Denial or Admission and Reason for the Violation:

The violation is correct as stated.

Beam 55-L-KZ: The cause of the noted condition was due to lack of a placing tolerance specifically applicable to beam reinforcing steel which led inspectors to mistakenly apply center to center spacing or rebar concrete cover tolerances to beam steel instead. 'ebar The maximum tolerance which was used by the inspectors was + 2".

Beam 38: The reason for mislocation of the reinforcing steel in Beam 38 is indeterminate, but was possibly caused by shifting of the rebar to clear the embed studs.

The concrete void indicated by "honeycombing" visible at the surface adjacent to the embed plate was apparently caused by aggregate being trapped on top of the bottom mat of reinforcing steel. Post placement inspection had not yet been performed in this area.

These post placement inspections are underway in other areas throughout the plant, not already inspected, to detect this type of defect.

Corrective Ste s Taken and Results Achieved:

An engineering evaluation is underway to establish beam reinforcing steel tolerances and to determine the acceptability of beam steel in previously constructed beams based on the

+ 2" maximum tolerance used by inspectors.

Beam 38: Permanent )Vaiver, P)V-C-5825 Rev. 1 was approved to accept the condition of the mislocated reinforcing steel of the subject beam "as-is".

Corrective Ste s Taken to Avoid Further Noncom liance:

Beam 55-L-KZ: Once the engineering evaluation has been completed and beam reinforcing steel tolerances are established, the criteria will be incorporated into the appropriate work and inspection procedures.

Beam 38: Additional rebar inspection steps have been implemented since this beam was constructed. No additional corrective steps are considered necessary.

Date When Full Com liance Will Be Achieved:

It is currently projected that full compliance will be achieved by May 31, l985.
