Attachment to CPRL Letter of Response to NRC Report RII: GFM/RLP 50-000/85-00-02 Re rted Violation:
10 CFR 50.55(f)(1) requires CPRL tc implement the quality assurance program described or referenced in the Preliminary Safety Analysis Report.
Section 1.8.5.3 of the PSAR (versus the Quality Assurance
- Program, as stated in the Notice of Violation) requires that measures be established and implemented for the selection and review for suitability of application of materials,
- parts, and equipment that are essential to the safety-relaxed functions.
Contrary to the
- above, on 3anuary 28,
- l985, the Inspector identified an undersized electrical c ble in DC panei D. -IASA. This cable (No. 10$ 21A-SA) 'vas a xwo-condo-x r
(/IO AWG that has a maximum ampere rating of 20 amps.
It was protected by a 100-ampere circuit breaker and supplied power to Auxiliary Transfer Panel lA-SA which had a loading of approximately 100 amps.
This cable had been installed and inspected in accordance with approved design documents and this unsatisfactory condition had not been identified.
This is a Severity Level Y violation (Supplement II).
Denial or Admission and Reason for the Violation:
The violation is correct as stated.
As complete information on load was unknown, an arbitrary size was chosen by HPES Electrical and was input into the cable routing system in order to assure an adequate route was available for the subject cable.
The cable card was inadvertently released xo the field prior to final verification.
Corrective Ste Taken and Results Achieved:
The subject
/710 AWG conductor was replaced with a /72 AWG conductor per Field Change Request (FCR-E-0052).
Corrective Ste s Taken to Avoid Further Noncom liance:
Administrative instructions have been issued to HPES Electrical personnel to ensure closer monitoring and control of cable sized by HPES.
In addition, a formal guideline procedure to place adequate control and verification requirements on cable sized by HPES is scheduled for issuance by April 30, 1985.
