Following is the written report of an event reported by phone to NRC Region II office (Mr. Jack Bryant) on June 5, 1979. The condition has also been reported by Carolina Power and Light to Region II. This report is made at the suggestion of Mr. Bryant to provide a complete report of the information required by 10CFR Part 21.

There was a misunderstanding of the intent of code require-ments as presented in NE4621.1 and Table NE4622.1-1 Sum-mer 1972 Addenda with regard to the sleeve to containment-liner insert weld. The welds in fifteen sleeves were not stress relieved.

Tube Turns Diion CHEME1RON CORPORATION (V) Date problem was confirmed to exist:

A determination was made on June 4, 1979 that the stress relief operation is a requirement and had not been per-formed on these sleeves.

(VI) Locations where the problem may exist:

Our initial review indicates no other nuclear power stations have been furnished similar subassemblies in an unheat-treated condition. A more thorough review is now being made and NRC will be advised within two weeks of the results of this review.

(VII) Corrective action:

Subassemblies that have not been installed are being returned at once to Tube Turns for stress relieving. The four units reported as being installed are being removed by Carolina Power and will be returned to Tube Turns for stress relieving. It is estimated that the first pl.eces will be returned to the fl.eld site by about June 18 and the balance about two weeks after receipt in Louisville.

(VIII) Advice to purchaser:

Ebasco, the Architect Engineer, and the owner, Carolina Power and Light, are aware of all of the information known regarding the conditions.
