INTEGRATED INSPECTION REPORT 50-400/97-08
NOTICE OF VIOLATION AND DEVIATION
Dear
Hr . Robinson:
On August 2,
1997, the
NRC completed
an inspection at your Harris reactor
facility.
The enclosed report presents
the results of that inspection.
During the six weeks covered by this inspection period, our inspectors
found
that your staff generally took a safety conscious
approach to oper ation of the
facility.
Our inspectors
found one violation of NRC requirements
and one
deviation from a commitment you made to the
NRC.
The violation is cited in the enclosed
Notice of Violation, and the
circumstances
surrounding the violation are described in detail in the
enclosed report.
Please
note that you are required to respond to this letter
and should follow the instructions specified in the enclosed
Notice when
preparing your response.
The
NRC will use your response,
in part, to
determine whether further enforcement
action is necessary
to ensure
compliance
with regulatory requirements.
The deviation is cited in the enclosed
Notice of Deviation,
and the
circumstances
sur rounding the deviation
ape described in detail in the
enclosed report.
The
NRC has concluded that information regarding the reason
for the deviation, the corrective actions taken
and planned to correct the
deviation and prevent recurrence
are already adequately
addressed
on the
docket in your supplemental
response
to violation 50-400/96-01
dated August
11,
1997.
Therefore,
you are not required to respond to the deviation unless
the description therein
does not accurately reflect your corrective actions
or
your position.
In that case,
or if you choose to provide additional
information, you should follow the instructions specified in the enclosed
Notice.
In addition,
an apparent violation was identified and was considered for
escalated
enforcement
action in accordance
with the "General
Statement of
Policy and Procedures
for NRC Enforcement Actions" (Enforcement Policy),
The apparent violation involved numerous deficiencies
associated
with your surveillance test procedure
program which in most cases
resulted in
your failure to meet Technical Specification Surveillance
