INTEGRATED INSPECTION
REPORT
NO. 50-400/96-06
Dear Mr. Robinson:
This refers to an inspection
conducted
on June
9 through July 20,
1996, at the
Harris Unit
1 reactor facility.
The subject inspection
included
a review of
design control
and plant changes
associated
with the spare
component cooling
water
pump
and spare charging/safety
injection pump.
Emergency diesel
generator
overload protective interlocks for the spare
component cooling water
pump described
in the Final Safety Analysis Report
and plant design basis
documents
were not installed in the plant.
Overload protective interlocks
described
in the plant design basis
document for the charging/safety
injection
pumps were also not installed.
Key interlocks which were installed to provide
redundancy
and separation
protection for the spare
component cooling water
pump
and spare charging/safety
injection
pump were removed without initiating
appropriate
design control measures.
The subject inspection report which
identified these
discrepancies
as
an apparent violation, was transmitted to
you by letter dated
August 7,
1996.
A closed predecisional
enforcement
conference
was conducted
in the Region II office on August 22,
1996, to
discuss
the apparent violation, the root causes,
and your corrective actions
to preclude
recurrence,
A list of conference
attendees,
along with slides
used
by the
NRC and licensee
during the presentation,
are
enclosed'ased
on information developed
during the inspections
and the information
provided
by your staff during the conference,
the
NRC has determined that
violations of NRC requirements
occurred.
The violations are cited in the
enclosed
Notice Of Violation (Notice)
and the circumstances
surrounding
the
violations are described
in detail in the subject inspection report.
Although
a
NRC review determined that these
issues
did not constitute
an unreviewed
safety question,
we are concerned
that emergency
diesel
generator
overload
protective interlocks were not translated
into drawings
by the design control
process
and
hence
were not installed in the plant.
Multiple opportunities
existed for you to identify and resolve this discrepancy prior to
NRC
identification.
The
NRC is also concerned
that this overload potential
and
the function of the overload interlocks were not described
in the electrical
section of the Final Safety Analysis Report.
We are also concerned
that you
removed the key interlocks which were installed to provide redundancy
and
train separation
protection without initiating appropriate
design control

measures,
resulting in plant design
documents that did not reflect actual
plant configuration.
You are required to respond
to this letter and should follow the instructions
in the enclosed
Notice when preparing your response.
In your response,
you
should document the specific actions
taken
and
any additional actions
you plan
to prevent recurrence.
Your response
should also address
your actions to
assure that
a broader design control problem does not exist.
After reviewing
your response
to this Notice, including your proposed corrective actions
and
the results of future inspection,
the
NRC will determine
whether further
NRC
enforcement
action is necessary
to ensure
compliance with NRC regulatory
requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter, its enclosure,
and your responses
will be placed in the
NRC
public document
room (PDR).
To the extent possible,
your response
should not
include any personal
privacy, proprietary,
or safeguards
information so that
it can
be placed into the
PDR without redaction.
