NOTICE OF VIOLATION

NRC

INSPECTION

REPORT

NO. 50-400/94-05

This refers to the inspection

conducted

by J.

Tedrow of this office on

January

15 - February

18,

1994.

The inspection

included

a review of

activities authorized for your Harris facility.

At the conclusion of the

inspection,

the findings were discussed

with those

members of your staff

identified in the enclosed

inspection report.

Areas

examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted

of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observation of

activities in progress.

Based

on the results of this inspection,

certain of your activities appeared

to be in violation of NRC requirements,

as specified in the enclosed

Notice of

Violation (Notice).

The violations are of concern

because

they indicate

recurring problems with the conduct of filter backflushing evolutions

and your

ability to identify problems with the plant computer

system within a

reasonable

time.

In December

1991, the operators failed to implement

procedures

properly for the backflushing of the recycle evaporator feed'ilter

which resulted

in increased

airborne activity in the Waste Processing

Building.

A.non-cited violation

(NCV 400/91-27-02)

was issued for this

incident.

In August 1992, the operators failed to implement procedures

properly to backflush the seal

water return filter which resulted

in a

challenge to the system relief valve.

A Severity Level

IV violation was

issued for this incident

on September

17,

1992.

With regard to the recurring

problems with the plant computer system,

a similar failure occurred in January

1992 when operators failed to detect

an

ERFIS failure.

The

NRC expects

licensees

to learn from previous failures

and to take effective corrective

actions.

Therefore,

you are requested

to give particular attention to your

response

relative to the identification of the root cause of these

problems

and your corrective action to prevent recurrence.

In addition, the enclosed

Inspection

Report identifies activities that

violated

NRC requirements

that will not be subject to enforcement

action

because

the licensee's

efforts in identifying and/or correcting the violation

meet the criteria specified in Section VII.B. of the Enforcement Policy.

You are required to respond to this letter

and should follow the instructions

specified in the enclosed

Notice when preparing your response.

In your

response,

you should

document the specific actions

taken

and

any additional

actions you plan to prevent recurrence.

After reviewing your response

to this

Notice, including your proposed corrective actions

and the results of future

inspections,

the

NRC will determine

whether further

NRC enforcement

action is

necessary

to ensure

compliance with NRC regulatory requirements.

In accordance

with 10 CFR 2.790, of the NRC's "Rules of Practice,"

a copy of

this letter and its enclosures will be placed in the

NRC Public Document

Room.
